Hi, I am Sara Grace Thress and I am a registered Licensed Clinical Social Worker in the state of California. I have a 10 year history of working as a medical and hospice social worker in acute care hospitals and homes. I have spent the entirety of my career providing crisis intervention, mental health guidance, long-term care planning support and appropriate resources to patient's and their families faced with navigating a terminal diagnosis. My true passion in this line of work is to meet each individual person where they are in their journey and to provide guidance, compassion and knowledge that will lead to a meaningful and empowered end of life experience for everyone involved.
I have an extensive history working within the healthcare system, alongside nurses, doctor's and surgeons. I understand the deep complexities of medical diagnosis and the numerous challenges of moving through the health care system in our world today. I am here to serve as your advocate both inside and outside of the hospital setting and to ensure that your voice and wishes are incorporated into your plan of care.
Death is a sacred experience and one that should be honored and celebrated within the bounds of your personal values and wishes. Together, we can plan ahead with the goal of easing stress, fear and anxiety to cultivate a beautiful transition.

As a practicing LCSW with 10 years of experience in the medical field, my work has centered on end-of-life care - helping individuals and families navigate anticipatory grief, complex emotions, and the tender, complicated terrain of dying with steadiness and humanity.

The paperwork and logistics around end-of-life planning can feel heavy and intimidating. It’s not always clear where to begin, even though these decisions matter deeply for you and the people you love.
I offer steady, compassionate guidance through each step, bringing clarity to what can feel overwhelming. From advance healthcare directives and POLST forms to conversations about DNR orders, choosing a healthcare power of attorney, and selecting a hospice agency, I’m here to help you navigate the process with confidence so you don’t have to do it alone.

Disposition Education
Burial, cremation, green burial, and water cremation - there are more options for caring for our remains than most of us were ever taught. Yet we rarely talk about them until we’re forced to.
I create a safe, grounded space to explore each option openly, answer your questions without pressure, and help you shape a plan that truly reflects your values, beliefs, and wishes - so your final arrangements feel intentional, not rushed.

Vigil Coordination
Vigil planning gives you the chance to shape your final days and moments with intention rather than leaving them to chance. I help you clarify what matters most, whether that’s being in a certain room of your home, having specific people present, incorporating meaningful rituals, or curating a music playlist that feels like you.
I work closely with you and your family to gently coordinate and carry out these wishes, creating a peaceful, supported experience that reflects your values and honors this sacred transition.

Medical Aid in Dying (MAiD)
For those who want to explore Medical Aid in Dying (MAiD), I provide clear information, thoughtful education, and grounded guidance through the process. During my five years as a hospice social worker, I attended and supported many clients and their families at the bedside during ingestion, so I understand both the clinical steps and the emotional landscape.
I help connect you with local physicians who support this option and remain present throughout each stage - offering steady support, advocacy, and care so you are never navigating it alone.

Aromatherapy
Aromatherapy offers a gentle, non‑invasive way to bring comfort and calm into end‑of‑life spaces. Sara Grace will use high grade essential oils to create a soothing atmosphere, ease emotional tension, and support a sense of grounding for the dying person and their loved ones. The purpose of using oils during the dying process is to enhance presence and connection during one of life’s most meaningful transitions.
