At Topkare, we believe death is a natural part of life and that conversations about it should not begin only when a crisis occurs. Education, honest conversation, and a clear understanding of our choices can replace some of the fear surrounding death with clarity, preparation, and intention.

We provide hospice care throughout the San Diego community, combining expert medical care and symptom management with emotional, spiritual, social, and holistic support. Our interdisciplinary team cares not only for the person receiving hospice, but also for the family and loved ones walking alongside them.

One of the things that makes Topkare unique is our commitment to caring for the whole person. Alongside traditional hospice services, we offer integrative therapies that may include massage and gentle touch, Reiki and Healing Touch, aromatherapy, acupuncture and acupressure, music and sound healing, reading, mantras, and other individualized practices that may bring comfort, connection, or peace.

But holistic care, to us, means more than offering complementary therapies. It means asking: Who is this person? What matters to them? What brings them comfort? What are they afraid of? What do they hope for? What would make this chapter of their life feel most like their own?

We believe there is no single right way to die. Some people find comfort in spirituality or ritual. Others find it in music, nature, family, humor, quiet, or simply being at home. Some want to talk openly about dying, while others do not. Our role is not to define a good death for someone else. Our role is to listen, educate, support, and honor the choices of the person and family in front of us.

That commitment to autonomy also includes supporting honest conversations about legally available end-of-life choices in California. We believe people deserve accurate information, compassionate support, and care without judgment or abandonment.

Our work continues beyond the death itself. We support families through grief and bereavement and seek to create opportunities within our community for conversations about death, dying, loss, caregiving, and what it means to live well while we are here.

Ultimately, Topkare Hospice exists to help people approach the end of life with more knowledge, more choice, less fear, and the freedom to make this deeply personal experience their own.

Topkare Hospice was born from a belief that we can do better when it comes to how we care for people at the end of life.

Topkare Hospice founder Jamie Hunt (center)

Topkare Hospice San Diego, CA

Our founder, Jamie Hunt, is a registered nurse whose years caring for critically ill patients in the intensive care unit shaped the vision for Topkare. Again and again, she witnessed families facing some of the most difficult decisions of their lives while in crisis - often without ever having talked about death, what mattered most, or what their loved one would want if faced with a life-limiting illness. Those experiences created a deep commitment to making end-of-life care more informed, personal, and compassionate.

That vision is strengthened by our Medical Director, Brian Graham, MD, who brings more than 20 years of hospice and end-of-life experience to Topkare. His role goes far beyond medical oversight. He is actively involved in patient care, meets personally with patients and families, and remains accessible when questions, concerns, or difficult decisions arise. We believe families deserve the opportunity to speak directly with an experienced physician who understands end-of-life care, takes time to listen, and can help them understand what is happening and what choices are available.

Holistice Hospice Care with Topkare

  • Topkare Holistic & Integrative Hospice Care

    Holistic & Integrative Hospice Care

We believe hospice should care for the whole person - body, mind, and spirit. In addition to expert medical care and symptom management, Topkare offers integrative therapies that may include massage and gentle touch, Reiki and Healing Touch, aromatherapy, acupuncture and acupressure, music, sound healing, reading, mantras, and other individualized approaches to comfort. What draws us to this work is recognizing that comfort means something different to every person. Sometimes relief comes through medication, and sometimes it comes through music, touch, a familiar scent, quiet presence, or connection. We want patients and families to have as many meaningful tools for comfort as possible.

  • Topkare Hospice San Diego CA Death Education & End-of-Life Conversations

    Death Education & End-of-Life Conversations

So much of the fear surrounding death comes from not talking about it until we are forced to. Our founder's years as an ICU nurse showed her what can happen when families are asked to make life-changing decisions during a crisis without knowing what their loved one would have wanted. That experience created a deep commitment to helping normalize conversations about death and dying. We believe knowledge creates choices. We want people to understand hospice, ask difficult questions, talk about what matters to them, and explore their wishes before a crisis makes those conversations urgent. Talking about death does not diminish life; it can help us become more intentional about how we want to live.

  • Experienced & Accessible Medical Leadership Topkare Hospice

    Experienced & Accessible Medical Leadership

Hospice families deserve more than medical oversight behind the scenes. Our Medical Director, Brian Graham, MD, brings more than 20 years of hospice and end-of-life experience to Topkare. He is actively involved with our patients and families, meets with them personally, and is accessible when questions or concerns arise. At the end of life, medical questions can be complicated and deeply personal. Having an experienced hospice physician who is willing to sit with families, listen, explain what to expect, and help them understand their options provides both reassurance and clarity. That combination of experience, accessibility, and personal involvement reflects the kind of hospice we want Topkare to be.

  • Personalized End-of-Life Care Topkare Hospice San Diego, CA

    Personalized End-of-Life Care

There is no universal definition of a good death. For one person, comfort may mean being surrounded by family and music. For another, it may mean quiet, nature, prayer, laughter, a beloved pet nearby, or simply remaining in their own home. We are drawn to deeply individualized care because hospice is not about imposing our idea of what someone's final chapter should look like. We listen for what matters most to each person - their values, relationships, culture, beliefs, fears, hopes, and preferences - and build our care around them. Our goal is for people to remain themselves throughout the dying process and to have as much voice and choice as possible in how they experience it.

  • Patient Autonomy & End-of-Life Choice including medical aid in dying with Topkare Hospice San Diego, CA

    Patient Autonomy & End-of-Life Choice

We believe every person deserves accurate information about their options and the freedom to make deeply personal end-of-life decisions according to their own values and beliefs. That includes supporting honest conversations about California's End of Life Option Act and medical aid in dying. Topkare respects the rights of eligible patients who choose to explore this option and believes that no patient or family should feel judged, abandoned, or unsupported because of the choices they make at the end of life. We do not believe our role is to decide what a good death should look like for someone else. Our role is to educate, listen, provide compassionate care, and remain present.

  • Family & Caregiver Support with Topkare Hospice

    Family & Caregiver Support

When someone is dying, an entire circle of people is affected. Hospice care is not only about the patient. Families and caregivers may be navigating exhaustion, uncertainty, anticipatory grief, changing relationships, difficult decisions, and emotions they have never experienced before. We are drawn to supporting the entire family system - helping caregivers understand what is happening, preparing them for what may come next, answering the questions they may be afraid to ask, and making sure they know they do not have to navigate this experience alone.

  • Topkare Hospice Grief & Bereavement Support

    Grief & Bereavement Support

Our relationship with a family does not end when their loved one dies. Grief is individual, unpredictable, and rarely follows a neat timeline. Through bereavement support and community grief offerings, we create space for people to experience loss without feeling that they need to grieve in a particular way or move on according to someone else's schedule. We believe grief is part of loving, and that people deserve compassionate support as they learn how to carry that love forward in a changed life.

  • Topkare Hospice Community Death Literacy & Changing the Conversation

Community Death Literacy & Changing the Conversation

We want to help create a community where death is not something we only whisper about after something has gone wrong. Part of our work extends beyond the bedside through education, community conversations, grief gatherings, and opportunities for people to become more comfortable discussing mortality, caregiving, hospice, dying, and loss. We believe becoming more comfortable with death can actually help us live more intentionally. When people understand their options and feel permission to talk openly about what matters to them, they are better equipped to advocate for themselves and the people they love. Music, sound, ritual, and other meaningful forms of connection can also be part of these conversations and part of the individualized experience we create at the bedside.
